Hydroxypropyl-beta-cyclodextrin in people
In people, hydroxypropyl-beta-cyclodextrin has shrunk a carotid plaque by more than a third in 35 days, reversed the yearly growth of coronary calcium, opened a critical coronary narrowing from 82 % to 24 % and relieved angina in nine of ten patients in a case series. Trials are running on three continents.
Plaque regression reports
| Year | Finding | Report | Detail |
|---|---|---|---|
| 2024 | Rapid carotid plaque regression | Case report, intravenous HPβCD | Carotid plaque reduced by 35.9 % on day 35 of the infusion regimen, with lower lipid and uric-acid levels and improved liver function.[1] |
| 2025 | Angina relieved, regression documented | Case series, HPβCD therapy | 18 of 20 patients with active angina (90 %) reported significant clinical improvement; objective regression of atherosclerosis in high-risk patients.[2] |
| 2025 | Better symptoms and quality of life in 12 weeks | Observational study, rectal HPβCD (Cavadex) | A high-risk group — prior heart attack, high coronary calcium, persistent angina — reported improved health, energy and cardiovascular symptoms, with angina reduced or resolved and exercise capacity enhanced.[3] |
| 2026 | Calcified plaque reversed | Multi-centre retrospective case series, high-viscosity HPβCD (Cavadex) | Coronary calcium progression reversed from +27.9 % a year to −10.5 % a year in an index case; a critical coronary stenosis fell from 82 % to 24 %; more than 90 % of symptomatic participants reported relief.[4] |
Human cells, human plaque
In blood cells from 76 patients undergoing coronary angiography, a hydroxypropyl cyclodextrin significantly reduced the monocytes' uptake of cholesterol crystals — the step that ignites the inflammasome[9]. In human atherosclerotic plaque tissue, the ring raised the oxysterols that switch on cholesterol export[10].
Registered trials

| NCT06813339 | UDP-003, a cyclodextrin built to remove 7-ketocholesterol | First-in-human, Cyclarity Therapeutics — recruiting since February 2025[6] |
| NCT04860960 | Intravenous HPβCD (Trappsol® Cyclo™) in Niemann-Pick type C1 | Phase 3, Cyclo Therapeutics[7] |
| NCT05607615 | Intravenous HPβCD in early Alzheimer's disease | Phase 2, Cyclo Therapeutics[8] |
A long human record
Hydroxypropyl-beta-cyclodextrin has been given to people for decades: intravenously as a parenteral excipient and a lipid-mobilising agent[11,12], and into the spinal fluid in Niemann-Pick disease, where it slowed neurological progression[13,14].
